If you are transferring from:
- a NHS hospital Trust to another NHS hospital Trust; a primary care trust; Mental Health Trust or an ambulance trust
- a Primary care Trust to a NHS hospital Trust; another Primary Care Trust; a Mental Health Trust or an ambulance trust
- a mental health trust to a NHS hospital Trust; another Mental Health Trust; a primary care trust; or an ambulance trust
- an ambulance trust to a mental health trust, primary care trust or another ambulance trust.
